A dedicated Care Planner serves as a single point of contact, developing customized care plans tailored to each client’s specific needs. The CareTeam is committed to making a meaningful difference in clients’ daily lives and overall well-being. Role Description The Care Planner and Business Development Manager is a full-time, hybrid role based in Cornwall, ON, with some work-from-home flexibility. This role is responsible for assessing client needs, developing personalized care plans, and serving as the primary point of contact for clients and their families. Day-to-day tasks include conducting in-home or virtual assessments, coordinating services with caregivers and healthcare partners, and monitoring care plans to ensure quality and continuity of care. In addition, the role includes business development activities such as building relationships with referral sources, promoting Right at Home Ottawa’s services, identifying new opportunities in the community, and supporting marketing initiatives. The position requires close collaboration with the CareTeam, local healthcare providers, and community organizations to optimize client outcomes and grow the service footprint in the Cornwall area and beyond. Qualifications Strong Planning and organizational skills to develop, implement, and adjust individualized care plans and outreach strategies. Excellent Communication skills, including active listening, clear written and verbal communication, and the ability to collaborate effectively with clients, families, caregivers, and community partners. Robust Analytical Skills to assess client needs, interpret clinical and non-clinical information, and make data-informed decisions about care plans and service offerings. Experience in Sales and relationship-building to grow referral networks, promote services, and support business development goals. Familiarity with Media Planning or basic marketing concepts to contribute to local outreach campaigns and community engagement activities. Background in healthcare, social services, care coordination, or case management is strongly beneficial. Post-secondary education in a relevant field such as healthcare, social work, business, or a related discipline is an asset. Proficiency with standard office software, CRM tools, and remote collaboration platforms. Ability to travel locally within Cornwall and surrounding areas; valid driver’s license and reliable transportation are preferred.
